Before you start
You must be a Yukon resident.
You will need:
- a valid Yukon health card (YHCIP), and
- the email or phone number you provided at your most recent vaccination appointment.
You can only use this service to view your own immunization records.
If your information is not on record, or your record is incorrect or incomplete, you will be not be able to use this online service. Visit your local health centre to get a copy of your immunization records.
