Apply for the Paid Sick Leave Rebate as a self-employed person

Use this service to apply for a rebate if you are self-employed and need to take time off work, including a 14-day self-isolation period due to COVID-19.

A doctor's note is not required. You can only apply once.

Before you start

You need:

  • A telephone number and email address so we can contact you;
  • a copy of your most recent Notice of Assessment from Canada Revenue Agency (CRA) to provide evidence of self-employment earnings;
  • A copy of the Income and Tax Benefit Return that provides evidence of self-employment income; and
  • a copy of your business licence dated before March 1, 2020, proof of your office location (such as a utility bill, or lease) or municipal business licence.

We will accept a digital photograph or different formats of documents (PDF, DOC, CSV or XLS).
