Assess yourself if you are sick with a respiratory illness

COVID-19 and other respiratory viruses are circulating in our community. Many people wonder what they should do when they get sick since lab-based PCR testing is no longer recommended for everyone who gets respiratory symptoms.

Use this self-assessment tool to help determine whether you should see a health care provider who can test you for COVID-19, or what your other options may be. You can complete this assessment yourself or on behalf of someone else if they are not capable.

This assessment is for all persons who have any of the following symptoms:

  • fever/chills
  • cough
  • shortness of breath
  • runny nose
  • sore throat
  • loss of sense of taste or smell
  • headache
  • fatigue
  • loss of appetite
  • nausea or vomiting
  • diarrhea
  • muscle aches
